Two copies of Form I-9 are included in the INS Handbook for Employers (Form M-274). Call the INS at 1-800-870-3676 to order the handbook. Form I-9 is also available at the INS Web site at www.uscis.gov. If you have questions about immigration-related employment matters, call the INS at 1-800-357-2099.
IRS Forms SS-4, W-2, W-3, W-4 and 1040 and related schedules and instruction can be obtained by logging on to the IRS Web site at www.irs.gov. Publication 15 (also known as Circular E) and Publication 926-Household Employer's Tax Guide are also available on the IRS Web site in PDF. You can also order tax forms, instructions and publications over the phone by calling 1-800-829-3676.
